What is the penoscrotal web or scrotal webbing Repair?
The penoscrotal web is a fold of skin from underneath the penis to the scrotum (sac), which lifts the scrotum. The penoscrotal web can give rise to the penis impression as shorter than the actual length. It can occur from birth as congenital abnormalities or as a result of tight circumcision. Men may want to correct the penoscrotal webbing because of short penis appearance, discomfort during exercise, painful sexual activities or difficulty putting a condom on the shaft of the penis. Penosecrotal webbing gives the formation of a ‘turkey-neck deformity’. This also results in a loss of junction of the penoscrotal angle and may result in an issue with sexual intercourse or psychological problems.

What are the operations for the Penoscrotal web?
Mild scrotal webbing can incise transversely and close the wound gap vertically, but dog-ear edges must be excised to prevent an unattractive appearance.
Mild webbing may be operated with a single or double Z-plasty.
Moderate webbing can result from the excision of triangle skin at the penoscrotal area with a V-Y-plasty. Scrotal lift, scrotoplasty, or scrotum reduction surgery may be required separately.
Excess skin can be excised along the midline for more severe webbing, with a Z-plasty at the penoscrotal junction to prevent recurrent scar shrinking.
Double V penoscrotal repair is also described in the medical literature.
Scrotal reduction surgery is not recommended in young patients, which may disturb the testes' temperature regulations and lead to infertility.

What is the aftercare of Penoscrotal web repair?
You might experience pain for a few days after the penoscrotal web repair, but simple painkillers help to reduce the discomfort.
Passing urine is usually fine after the procedure.
There will be swelling and bruising of the shaft of the penis and the scrotum for 2 to 3 weeks after the operation. This swelling will then lessen gradually, and you should not be worried.
Bleeding after penoscrotal web repair is uncommon; however, it can be stopped by putting pressure or a bandage.
A dressing (bandage) is applied around the wound line to prevent swelling. A bandage with self-adhesive stretch gauze will remain for three days (Cobain).
The wound will take 2-3 weeks to heal, and you may return to work when you are comfortable enough. It usually takes 2-3 days.
Most people require at least 2-3 days off work except manual workers. You must not do heavy manual labour for 1-2 weeks.
You should gently rinse the wound daily after ten days if the wound is dry and shower regularly.
You must avoid going to the gym or lifting weights for two weeks.
You should also refrain from any sexual activity for 4-6 weeks after the procedure to avoid the breakdown of the wound.
What are the risks of Penoscrotal webbing operation?
The wound is commonly bruised around the line of the incision. Some patients may have bleeding, infection, wound gap, keloid scarring, redo operation and poor outcomes. It is essential to consider this operation to weigh the benefits versus risks.
